I understand that if I am hired for a position that will require or is reasonably likely to entail the operation of a City vehicle, I will be required to provide a copy of a valid state driver’s license to the City and that said license with be used to check my driving check and other lawfully authorized information.

I understand and agree that if offered a job, the first six months of employment shall constitute a probationary period. By my signature I agree that the employment relationship between myself and the City of Westminster is not covered by any guarantees, obligations or contracts of employment, either expressed or implied. I further understand and agree that I have the right to terminate my employment at any time. The City of Westminster retains the right to terminate my employment for any reason not otherwise unlawful during the probationary period.
